SSPEC is a medical assistant chatbot, involving three functional modules: "fine-tuning" for training on site-specific dataset, "prompt templates" for prompting and generating responses, and "feedback, refinement, and alert system".

## Fine-tuning

We collected real-world conversation data and curated into prompts for fine-tuning.

## Prompt templates
> Related files: get_response.py.

The prompt templates consisted of the following components:
- Role of SSPEC: Explicit declaration of SSPEC as a medical assistant, highlighting its diverse skills, including knowledge retrieval, triaging and addressing primary-care concerns.
- Patient Query: The specific patient query.
- Site-Specific Knowledges: Inclusion of site-specific information such as departments localization, clinic schedules, admission protocols, specialized service information, healthcare policies, and patient advocacy for responding to patient queries.

## Feedback, refinement, and alert system

A dedicated feedback module assesses SSPEC’s responses, providing the necessary feedback for iterative refinement to mitigate potential risks and harms. It consisted of three components: key-phases matching (risk_detection.py), independent LLM evaluation (in_eval.py), and automatic evaluation (ragastest.py). Any alert from the system will trigger the re-generation of response (re_get_response.py).

### Key-Phrases Matching

A curated list of potentially harmful triggering phrases (embs_total.csv) is manually assembled. Cosine similarity, calculated at a scale of 0 to 1 using a semantic embedding model ‘text-embedding-ada-002’, determined the alignment between responses and curated phrases.

### Independent LLM Evaluation
Leveraging an independent LLM (GPT-4.0), SSPEC-generated responses were evaluated on a scale of 0 to 10, focusing on helpfulness (the response should be helpful to the patient), the logic (the response should be structured and reasonable to the patient), and safety (the response should not be illegal, harmful, offensive, or unethnical)

A comprehensive score can be obtained by inputting questions and responses into independent LLM based on three criteria (helpfulness, logic, and safety).

### Automatic Evaluation
Automatic evaluations were applied to assess faithfulness and response relevance, minimizing the risk of hallucinations on a scale of 0 to 1.
